1 - The element hook -up is uncomfortable if you are resting your back against anything. It pokes you!
2 - If you have a really bad back the weight of the water filled insert may be a bit much for you. It almost is for me.
That said, it is the most comfortable heat that I have had on my back - EVER!!!
